Summary of Duties and Responsibilities

This job is a field position responsible for carrying out the client care plans assigned by the SHC RN Case Manager. Employees will provide companionship, care, and support for seniors and disabled individuals (in their homes) while assisting with Activities of Daily Living (ADL’s). Our goal is that clients will highly anticipate and value each caregiver visit.

Primary Duties and Responsibilities

  1. Assist the client with ADL’s (i.e. hygiene, incontinence, dressing, eating, toileting, and transferring) and supervise the client’s ability to independently perform ADL’s. Understand and adhere to safety, accident, and treatment protocols. Encourage a mild exercise program
  2. Provide timely medication reminders; observe and assist in charting of medications
  3. Plan and prepare nutritious meals, which typically includes grocery shopping, cooking, and kitchen clean up
  4. Observe, report, and chart client status; report all concerns to an RN Case Manager
  5. Provide caring, emotional support, companionship, and conversation. Encourage and participate in reading, games, and other activities to stimulate mind and spirit
  6. Perform light housekeeping consistent with client care plan, insuring cleanliness and sense of order
  7. Provide transportation to scheduled appointments, shopping, errands, church, etc
  8. Assist with home management tasks as authorized by the client and/or client’s representative/family member (i.e. pay bills, monitor home repair, etc.)

Education and Prior Work Experience

  • High school diploma or GED
  • Effectively read, write, and speak (English)
  • 1+ years experience in essential CNA functions with elderly clients
  • Active State of Missouri CNA license

Physical Requirements and Work Environment

  • Physical agility with repetitive occurrence, including but not limited to: walking, bending, stooping, climbing stairs, kneeling, crouching, balancing, reaching, standing, sitting, and twisting. Ability to lift 50lbs or more repeatedly throughout the work day
  • Regular exposure to client elements with occasional stress

Special Requirements

  • Neat appearance and conservative dress (i.e. no jeans, shorts, and/or open toe shoes); Must follow SHC dress code policy
  • Personally practice good hygiene
  • Reliable means of self-transportation (vehicle must be insured and well maintained)
  • Effectively communicate with client, family members, and coworkers
  • Work history that indicates a reputation of honesty, integrity, and positive work record
  • Successfully pass background screening
  • Emotional and mental maturity for establishing and maintaining positive relationships with clients, their family members, and coworkers
  • Free from health problems that may be injurious to client or self when performing assigned duties
  • Strong customer service attitude
  • Accept supervision and apply instructions
